No plan yet? Most people can join a Medicaid health plan for free. Call New York Medicaid Choice at 1-800-505-5678, or NY State of Health at 1-855-355-5777 if your Medicaid case is with NY State of Health. You can still send this form and we will talk it through.

Long-term care plans (MLTC and MAP) cannot get a food box through this program, but other help may be possible. With Medicare and Medicaid, it depends on how your Medicaid is set up. We will check with you.

Not sure your health condition qualifies?

Many conditions can qualify, not only diabetes, heart or kidney disease and pregnancy, and so can people who use Medicaid services often, such as frequent emergency room visits or hospital stays. These usually need a short attestation from your doctor. We provide the attestation paperwork for your doctor to sign.
